Privacy Choices

Clear paths for access, correction, communications, and account closure.

Start with your Care Organization

Your healthcare provider, college, university, health center, or other Care Organization controls the clinical or education record. Contact that organization for time-sensitive record access, corrections, restrictions, confidential communications, or questions about its Notice of Privacy Practices. ozehr will assist the Care Organization as required.

Choices available in OzEHR

Manage communications

Open Profile to enable or disable optional SMS reminders. You can also reply STOP to an SMS, change device notification permission in system settings, or ask your Care Organization to update communication preferences.

Access or share records

Use Records in OzEHR to view available records, request a formal copy, or send selected records to an authorized recipient. Your Care Organization may verify identity or apply a legally permitted fee.

Request a correction, restriction, or disclosure information

Use the secure messaging or records-request workflow, contact your Care Organization, or email contact@ozehr.com. The portal's Privacy & Disclosure Log shows available disclosure history.

Close your OzEHR account

In the mobile app, open Profile → Privacy & Data → Request account closure. The request is recorded for review, active access is closed after verification, and information not subject to care, legal, security, audit, or retention obligations is deleted or de-identified where applicable.

Review privacy documents

Open On Call → Privacy & Data to read this Privacy Policy, review Privacy Choices, and open the Terms of Use from within OzOnCall.

Manage device permissions

Change notification permission in system settings. OzOnCall requests microphone access only when an authorized user starts an Oz Scribe workflow after required encounter consent is recorded.

Access or correct workforce information

Contact your Care Organization administrator for access to or correction of your workforce profile. You may also email contact@ozehr.com without including patient information, and we will coordinate through a secure channel when needed.

Deactivate your OzOnCall account

Open On Call → Privacy & Data → Request account deactivation. The authenticated request is recorded as an auditable task for a separate authorized organization administrator. Access is disabled only after verification; retained clinical documentation, audit evidence, legal holds, and other required records are not erased.

What account closure or deactivation means

Closing an OzEHR app or portal account, or deactivating an OzOnCall workforce account, prevents future account access and revokes active sessions after the request is verified and approved. It does not automatically delete the medical, education, workforce, audit, or security records maintained by your Care Organization or ozehr. Records that must be retained remain protected and available only as permitted by law, contract, and the Care Organization's policies.

No sale or mobile tracking opt-out is required

ozehr does not sell personal information or PHI, use PHI for targeted advertising, or use the OzEHR or OzOnCall mobile apps to track people across other companies' apps and websites. If you believe information was used inconsistently with this statement, contact us so we can investigate.

How we verify and respond

To protect health information, ozehr or the Care Organization may verify identity, account ownership, relationship, and an authorized agent's authority before acting. We will acknowledge a request and respond within the period required by applicable law. If a request is denied or limited, the responsible organization will explain available review or appeal rights where required.

Submit a request outside the app

Email contact@ozehr.com, call 775-239-9991, or mail ozehr LLC, 732 S 6th St, Ste N, Las Vegas, NV 89101. Include the type of request and the name of your Care Organization, but do not email sensitive medical details. We will use a secure channel if additional information is needed.

Questions and complaints

Read the full Privacy Policy or contact your Care Organization. If HIPAA applies, you may also complain to the U.S. Department of Health and Human Services Office for Civil Rights. ozehr and your Care Organization will not retaliate for a good-faith privacy request or complaint.
